Pain Relief in Pregnancy

What is it?

Pethidine is a narcotic, controlled drug, which was introduced in 1939. It’s most commonly given intramuscularly by injection into the thigh or buttock, although it can also be administered intravenously. It takes around 20 minutes to take effect and lasts for 2-4 hours. It can be re-administered, but as it affects the baby, you should be wary of having repeated doses.

How will it make me feel?

Many mums-to-be have reported that although pethidine helped them relax, by making them sleepy, it didn’t relieve the pain. Therefore they felt out of control during labour. However, as it can alter mood, it can sometimes work very well by enabling women to unwind and get some rest, while allowing labour to continue by relaxing the muscles of the cervix and womb. Different doses are given depending on weight, stage of labour and degree of discomfort, some women prefer to have ‘half a dose’, so they don’t feel too sleepy during labour.

Does it give effective pain relief?

Talk to a group of mums and they’ll each have a different answer. It can depend on when it was given and the amount administered. The normal dose is 50-100mg, which is only 1-2ml. Many women find it more effective when the contractions aren’t at their strongest, that is, closer to the beginning of the first stage. But this raises the problem of what to do once the pethidine has worn off. To be lying down in labour from such an early stage isn’t ideal and can slow things down. However, it’s also important to pace yourself, to be leaping out of bed in early labour and striding around the room will leave you feeling exhausted. Some women do have long labours, and despite using methods such as breathing techniques and soaking in a warm bath to relief the pain, they need something to help them rest, which is where pethidine is useful.

How does it affect the baby

It crosses the placenta and can make the baby sleepy, even after the birth, which may be serious if the injection is given close to the birth itself. Some babies with breathing difficulties will need an injection to reverse the effects of the pethidine. Others are slow to suck as a result of the pethidine and can have difficulties breast-feeding in the first few days. There’s also some evidence to suggest that newborns who receive an opiate drug through their mum’s bloodstream may be at an increased risk of addiction in later life.

The advantages

It can be given by midwives, so it is quickly available, unlike an epidural, which has to be sited by an anesthetist. Tension and anxiety can sometimes hold back labour, and a small dose may be just enough to help you relax. For some women, it’s a really effective form of pain relief.

The disadvantages

Apart from the effect it might have on your baby, you may feel out of control if you feel sleepy but still in pain. It can also be difficult to judge when to use pethidine, as it shouldn’t be given within two or three hours of the birth. One of the potential side effects is that it can make you sick, although an additional drug can be given to help prevent this. Another downside is that you’ll probably be restricted to bed as you’ll feel so sleepy, and you shouldn’t use a birthing pool.

“I hurt!” is one of the most common things a doctor hears. Most of the people I saw in 44 years of practice had failed to receive lasting pain relief – conventional or alternative, integrative or complementary. Often I found just listening to a patient I’d not only hear how they got hurt but quickly they’d tell me what didn’t work – And then they’d tell me what helped them!

Many times I’d find that gentle pressure or a slight sidewise-movement over their painful spots would relieve the trouble quickly, but briefly. These were ‘clues’ telling me what therapy to do next. Sometimes simple soft-tissue movement, simple naturopathic movements (without painful ‘crunching’), taping a special magnet over selected spots, and off they’d go: feeling MUCH better and not needing to return unless pain returned! Sometimes a simple touch upon one ear (the warmth of a colored felt-marker on THE spot could be enough!). It’s important to determine if there’s left/right handedness involved. When acupuncture is indicated, there are needle-free techniques; for more difficult conditions our THREE-POINT method can relieve and/or eliminate deep-seated problems immediately! NO NEED TO ‘MARRY’ ME FOR THREE TIMES A WEEK VISITS… if you don’t feel better immediately and I can’t rid you of your pain in 4-5 visits, then I’m not going to do it in 105 visits. My goals: pain relief NOW on less medication than you were using before and putting you in control of your own treatment.

The best results come from those who have never-ending pain. 24/7 pain relief is usually immediate, sometimes delayed for up to 8-10 hours, but always THE SAME DAY. If you’re a chain-smoker, plan to QUIT SMOKING in one twenty-minute (drug-free!) visit: but remember, I don’t sew shut your lips! No one but you puts that cigarette in your mouth… we need to change your lifestyle, and that’s the TRUTH!

If your pain comes-and-goes, it’s tougher to get rid of. If you have a migraine attack or tic-douloureaux once a month and it’s gone after one treatment, don’t think it’s over. The first time you eat chocolate or peanuts or drink alcohol, you may have another attack. I find MOST such attacks are sensitivity- or allergy-related; we need some good detective-work here plus your willingness to stop whatever we find is the cause of the trouble! The curious thing about some of these treatments: there may be what I choose to call ‘reverse tolerance’. That is, they can provide relief for 3-6 months at a time, then pain may return, and a single treatment then provides relief for another 3-6 months… or it just goes away until the ‘next time’ you slip on a banana peel or whatever…

If that’s the way you might respond, there is usually some structural or physical condition underlying. We’ll go ‘back to basics’: making sure it isn’t unequal leg-length; maybe it’s narrowing of bone where a nerve passes by or there’s scar-tissue from a prior injury or surgery. Or (worse-yet) no one paid attention to ligaments you may have overstretched or ‘pulled’ a long time ago. Herniated disc? Normally, that does NOT need surgery in over 90% of cases! (I speak from personal experience here.)

I can’t ‘undo’ damage from bone rubbing on bone. But most persons can expect to live fairly well several months pain-free; then a single ‘tune-up’ visit and away they go. Typically, such persons may need referral to a specially-trained MD or ND for 6-8 specific non-steroid prolotherapy treatments (approximately a month apart) and surgery may well be avoided. Many people have been told their problem’s “in their head”. Well yes, our brains do run everything: but I maintain that SYMPTOMATIC MEDICINE IS NOT GOOD MEDICINE… nothing beats a good doctor who makes a good diagnosis and then selects the best treatment. Ridding you of the cause gets rid of your symptoms.

I’ve been a user of Auriculotherapy for over 35 years now: In my opinion this particular form of treatment makes almost everything else work better… For that reason, if you’re not better after your first visit you may not be in the right doctors’ office. Yes, whether treating dogs, horses or humans: we have made HUGE advances. Many of us just need to slow down a bit and ‘listen tightly’ to what you tell us. There are still those among us who are not treating just to collect the fee (sometimes, money can be a root of ‘evil’). You should be comfortable asking your own doctor about what ails you and he/she should answer you.
